Certificate of Insurance for EACs

In order for any Exhibitor Appointed Contractor (EAC) to provide any services at this show, the EAC must use the link below to submit a valid certificate of insurance 30 days prior to show start. It should be prepared by an insurance agent and include the following data:

  1. PRODUCER: Insurance Agent / Broker who issues certificate.
  2. NAME OF INSURED: Must be the legal name of contracting party.
  3. TYPES OF INSURANCE: Must include types required by contract.
  4. FORM OF COVERAGE: Must be "occurrence" form of coverage.
  5. DESCRIPTION OF OPERATIONS/LOCATIONS/VEHICLES/EXCLUSIONS ADDED BY ENDORSEMENT/SPECIAL PROVISIONS: Helicopter Association International (“HAI” or “Show Management”), Helicopter Foundation International (“HFI”), GES, the City of Anaheim, the Anaheim Convention Center, and their respective officials, officers, directors, affiliates, subsidiaries, agents, employees, representatives, contractors, and consultants are named as additional insureds under all liability policies for the entire duration of HAI HELI-EXPO 2020 in Anaheim, CA (move-in: 01/22/2020 through move-out: 02/01/2020). A waiver of subrogation is included under all liability policies. All coverage is primary and non-contributory, written on an occurrence basis, and issued by a company authorized to transact business in the State of California.

  7. POLICY EFFECTIVE DATE: Must be prior to or coincidental with the first day of Exhibitor Move-In.
  8. POLICY EXPIRATION DATE: Must be on or after the last day of Exhibitor Move-Out.
  9. LIMITS OF INSURANCE: Must be the same or greater than required by contract.
    • Commercial General Liability Insurance covering all operations by or on behalf of EAC with limits of not less than one million U.S. dollars ($1,000,000) per occurrence and two million U.S. dollars ($2,000,000) aggregate per policy for Bodily Injury (including death) and Broad Form Property Damage. The policy shall include coverage for liabilities arising out of Premises, Operations, Independent Contractors, Products, Completed Operations, Personal & Advertising Injury, and Contractual Liability;
    • Comprehensive Automobile Liability Insurance covering owned, hired, and non-owned vehicles with a minimum bodily injury (including death) and property damage limit of one million U.S. dollars ($1,000,000) per occurrence. Such insurance shall include coverage for loading and unloading hazards;
    • Workers’ Compensation Insurance, as required by California statutes, covering all of EAC’s employees and agents (EAC shall require all sub-contractors to also obtain Workers’ Compensation Insurance as required by California statutes); Employers’ Liability Insurance with limits of not less than one million U.S. dollars ($1,000,000) for any one accident, one million U.S. dollars ($1,000,000) for disease each employee, and one million U.S. dollars ($1,000,000) disease limit; and
    • Umbrella Liability Insurance covering excess liability over all General Liability, Automobile Liability, and Employer’s Liability policies with a limit of not less than one million U.S. dollars ($1,000,000) per occurrence. The policy shall be on a following form basis with respect to all underlying coverages. EAC is responsible for payment of any retention or deductible.
  10. AUTHORIZED REPRESENTATIVE: Must be signed (not stamped) by an authorized representative of Producer.
