Certificate of Insurance for EACs

In order for any Exhibitor Appointed Contractor (EAC) to provide any services at this show, the EAC must mail in a valid certificate of insurance to the address below 30 days prior to show start. It should be prepared by an insurance agent and include the following data as per this sample:

  1. PRODUCER: Insurance Agent / Broker who issues certificate.
  2. NAME OF INSURED: Must be the legal name of contracting party.
  3. TYPES OF INSURANCE: Must include types required by contract.
  4. FORM OF COVERAGE: Must be "occurrence" form of coverage.
  5. NAME ADDITIONAL INSUREDS: Helicopter Association International (HAI), Helicopter Foundation International (HFI), Global Experience Specialists, Inc. (GES), the Las Vegas Convention and Visitors Authority, and the City of Las Vegas.

  7. POLICY EFFECTIVE DATE: Must be prior to or coincidental with the first day of Exhibitor Move-In.
  8. POLICY EXPIRATION DATE: Must be on or after the last day of Exhibitor Move-Out.
  9. LIMITS OF INSURANCE: Must be the same or greater than required by contract.
    • Commercial General Liability covering all operations by or on behalf of EAC with limits of not less than one million U.S. dollars ($1,000,000) per occurrence, two million U.S. dollars ($2,000,000) aggregate per policy for Bodily Injury and Property Damage including Personal Injury, Completed Operations, Products Liability, and Broad Form Contractual Liability;
    • Comprehensive Automobile Liability covering any and all automobiles including owned, hired, and non-owned vehicles with a combined single limit of not less than one million U.S. dollars ($1,000,000);
    • Workers' Compensation Insurance, as required by law, and Employers' Liability Insurance with limits of not less than one million U.S. dollars ($1,000,000) for any one accident, one million U.S. dollars ($1,000,000) for disease each employee, and one million U.S. dollars ($1,000,000) disease limit; and
    • Umbrella Liability covering excess liability over all General Liability, Automobile Liability, and Employer's Liability policies with a limit of not less than one million U.S. dollars ($1,000,000) per occurrence. The policy shall be on a following form basis with respect to all underlying coverages.
  10. AUTHORIZED REPRESENTATIVE: Must be signed (not stamped) by an authorized representative of Producer.
